Who can get Ayushman card made?

Step 1
If you want your Ayushman card to be made, then first go to the official website of the scheme https://pmjay.gov.in/

After this, click on the option of 'Am I Eligible' given here and then enter the captcha code given on the screen

Now enter your 10 digit mobile number and then get it verified by filling the OTP received on this number

Then login by filling the captcha code and choose the scheme

After this, choose your state and also select 'Sub Scheme' and select your district

After this, go to 'Search by' and choose a document such as Aadhaar card

Then fill the Aadhaar number and fill the captcha code given on the screen

Now finally click on Search
