"Brain death" is a medical term often surrounded by misconceptions. Many people confuse it with a coma, believing the patient might eventually regain consciousness. However, in medical science, brain death and coma are entirely different conditions. Brain death signifies that the individual's brain has completely and permanently ceased to function.

Dr. (Prof.) Daljit Singh, Vice Chairman and HOD of Neurosurgery and Unit Head of Neuro-intervention at Max Smart Super Speciality Hospital, Saket, explains that at this stage, there is no possibility of the patient recovering or returning to life. Although the heart may appear to beat and the chest may seem to rise and fall, these signs are sustained solely through the use of ventilators and other life-support systems.

What is the difference between brain death and a coma?
Experts explain that in a coma, the brain does not shut down completely. While the patient remains unconscious, some brain activity persists, and there is a potential for improvement through treatment. In contrast, with brain death, both the brain and the brainstem permanently cease functioning. The brainstem controls vital bodily functions such as breathing and heartbeat; therefore, an individual is considered dead following brain death.

How is brain death confirmed?
A rigorous medical procedure is followed to declare brain death; it is not a decision made by a single doctor. Doctors first verify whether brainstem reflexes have completely ceased. Subsequently, an apnea test is conducted to determine if the patient can breathe without mechanical assistance. In some instances, tests such as an EEG or a brain blood flow scan are also performed to confirm the absence of any remaining brain activity. Before a final decision is made, two separate doctors conduct examinations at different times to confirm the diagnosis.

How is organ donation possible after brain death?
Following brain death, the patient is kept on a ventilator for a period of time. During this time, the heart continues to beat, ensuring that blood and oxygen reach the body's organs. This keeps the organs healthy and suitable for transplantation. This is the crucial window when organ donation can take place. If the family consents, doctors initiate the process of transferring organs—such as the heart, lungs, liver, kidneys, and pancreas—to patients in need.

Why is organ donation important?
When a person suffers brain death, medical science cannot restore their life. However, their healthy organs can save the lives of other patients. This is why organ donation is considered one of the greatest acts of giving life. Although it is an incredibly difficult decision for a family to make, it can bring new hope and a new lease on life to other families.
